Admissions Process

Preadmission

Individuals speak with our intake coordinator, in order to assess as to whether individuals are appropriate for treatment. A mini assessment is conducted over the phone to assess appropriateness for admission into our facility. Our intake coordinator may speak with individuals and families to help with preadmission issues. Interventions via phone are often conducted, and our team ensures that all questions are answered prior to admission. Any special needs to be accommodated are discussed, and it is our specialty to provide help for families. We prefer to involve family in the preadmission process, if needed, in order to help with a smooth transition into treatment.

Admission

Clients receive a full intake, in order to discern unique issues that will be utilized throughout the treatment process. Clients meet with the case manager to complete an initial intake assessment. Clients also meet with the medical director and medical team to receive assessments, as well. Clients are introduced to their therapist, entire staff, and peers. This is done to ensure that all psychological, psychiatric, medical, and case management issues are immediately addressed upon admission. Clients receive an orientation packet and some incidentals they may need while in treatment. We pride ourselves in providing a full scope of assessments, in order to assess the nature of an individual’s problems in need of treatment.

Aftercare Plan

Aftercare planning can often be a tricky process, as some clients require less treatment and other clients require more treatment. When treatment is close to being completed, clients are provided with a discharge date, and they are helped to tour halfway houses, if they intend living at a halfway house in south Florida. Other clients may choose to return home to independent housing, or they may choose to return to their hometowns and live in halfway. Clients are helped through every step of the process to help in the discharge planning process. Clients are helped to set up psychotherapy, psychiatric, medical, vivitrol provider, and outpatient treatment center appointments with a date and time of appointment, prior to discharge. Clients receive a letter and certificate of completion, as well. Clients are encouraged to follow up and let us know how they are doing, as we take much care in how clients are continuing to progress post treatment. We love to have former clients who are sober come back and share their stories, “How they did it,” and former clients who wish to share their stories with current clients must sign confidentiality agreements and make an appointment with the program director to do so.

Treatment Plan

After the admissions process, clients meet with their therapists, in order to conduct a biopsychosocial assessment. The intake, biopsychosocial, psychiatric, history and physical assessments are all used in the formulation of the treatment plan. A Master Treatment Plan is created to help clients address unique issues specific to clients that will help treat mental health issues and substance use disorders. Clients are assigned a therapist and continue to meet with therapists and attend groups, while working on issues that are uniquely identified in the treatment plan. Any new issues that arise during the course of treatment will be added to the treatment plan and will continue to be addressed.
